Insurance

 

Services may be covered in full or in part by your health insurance or employee benefit plan.  Insurance usually covers most of the cost per visit with an average co-payment of $25-$35.  Your insurance carrier will be able to give you a quote of the exact amount of your co-payment.

 

Please check your coverage carefully by asking the following questions:

 

• Do I have behavioral health insurance benefits?

• Do I need special authorization under my policy?

• How much is my co-payment?

• Is there an annual deductible?

Session Times/Rates

A session is billed according to the amount of time we spend working together.  Your insurance carrier will cover a percentage of each session with an assigned co-payment according to the details of your policy.  Co-payments are due at the beginning of each session.  The following is a list of session times:

 

60-75 minute intake session

60 minute individual session

45 minute individual session

45 minute family session/couples counseling session

45 minute session on behalf of established client

30 minute individual session

60 group therapy session

 

 

Reduced Fee

Reduced fee services are available on a sliding scale and on a limited basis for applicants who meet financial qualifications.  Please be prepared to complete a financial agreement and provide your two most recent pay stubs.

 

Payment

Cash and check accepted for payment.

 

Cancellation Policy/Missed Appointment Policy

Please give 24 hours notice if you need to reschedule your appointment to avoid a late cancellation fee. 

In order to give you and all my clients the best possible care, I request that you review my policy regarding missed appointments.  A missed appointment is when you don't show up for an allotted appointment time, without a phone call or cancellation notice of at least 24 hours.  Please remember that I have reserved appointment times especially for you.  Therefore, please give me at least 24 hours notice in order to reschedule your appointment.  This will enable me to offer your cancelled time to another client who may be in need of an appointment.
